FCQ Optional Question Request Form

Deadline for submitting FCQ Optional Question Requests is October 1 in Fall terms and March 1 for Spring terms, because this is when we begin printing FCQs. However, we will always accept and process your optional question requests as long as we have not yet actually printed your FCQ forms -- so please feel free to submit 'late' requests. Optional questions are not available during Summer terms.

Both the paper FCQ form and the online FCQ have room for 24 optional questions of your choice, with a 150-character limit per question. If you know which questions you want to use, enter their ID numbers here. The ID numbers of nearly 900 questions that have been used at one time or another are in the Optional Question Catalog. If you click here a separate browser window will open so that you can see the form and the catalog at the same time.

You can search for optional questions you have used in the past 5 years.

You can also search the optional questions for particular text strings.

The optional questions will appear on your forms in order from 1-24.
